how do you make lists?

How do you make lists? All you guys have these nice neat lists and I don't.IE to join the RP you have to fill out a form how is it done I'm not up on all this stuff help!!

For an rp they usually post a form like this:
Form:
Name:
Gender:
Rank:
Description:
Personality:
And you fill it out with facts about you character,if you have any more question,feel free to pm me,I rp a lot on other sites
